Grade 7 and Grade 11

• Satisfactory result of the entrance examination and interview

• Form 138 with passing grades in all subjects and in conduct

• Photocopy of Birth Certificate

• Photocopy of Baptismal Certificate (for Catholics)

• Certification of good moral character from the Principal/Adviser/Guidance Counselor from school last attended

• 3 pcs. 1x1 and 2x2 recent ID picture

• 1 long brown envelope

• 1 long white folder

• Satisfactory result of the entrance examination and interview

• Form 138 with no failing grade in any subject and/or back subjects

• Photocopy of form 138 or permanent record for Grade 10 and Grade 12 enrollees
